Unfortunately, not enough students signed up for Mr. McClure to get the group rate that would have kept the cost affordable. We’re sorry.

If you have regularly scheduled work or outreach ministry during every performance, you will need to submit a DE-5 form. The DE-5 form is available on the Avenue. Keep in mind that if you revise a form that has already been approved, you will need to submit an entirely new request. The new request will replace your original request, not add to it, so be sure to fill out the new form completely.
